&lt;p&gt;This document does NOT say that the Ct will be lowered for testing breakthrough cases. It says they are only going to investigate breakthrough cases where positive test results were found with a Ct of &amp;lt;= 28. This could be the more severe cases. Perhaps they do not think it&amp;#39;s worth investigating cases found via a Ct over 28. &lt;/p&gt;
&lt;p&gt;Ok, the implication is there in plain site: Ct over 28 catches the iffy cases or is less than acceptably accurate. Also lower Ct is an indication of infection severity. &lt;/p&gt;
&lt;p&gt;Just after the election was decided, the WHO issued a vague recommendation that labs use a lower threshold. No number given. No Ct threshold was ever established anywhere on earth. We all know that. We all know that the PCR test starts to go funky after about 18 cycles, returning a higher amount of false positives as the Ct increases. There were tests, reported here and elsewhere, showing that at 35 cycles the chances of a false positive were overwhelming. Yet nothing was done, and even the docs quoted in the NYT article last summer wanted to keep the Ct at 35, which is still foolishly high. &lt;/p&gt;
&lt;p&gt;At some point some people hypothesized that the Ct where the virus was detected would be an indication of viral load, and thus a marker of illness severity. I really don&amp;#39;t know if that ever went anywhere, even as far as including the Ct used with the test results. I really doubt if the labs run the test for some smaller number of cycles, check to see, then run it some more if nothing is found, check again, then run it for further cycles. Lather, rinse, repeat. To the best of my knowledge, not a single study has been done that even tried to correlate Ct, positive results, and apparent illness level ... although around this time, the term &amp;quot;pre-symptomatic&amp;quot; was created, as asympt = false positive.&lt;/p&gt;
